The supply chain assessment forms were built by Supply Chain Sustainability LLC and we invite you to visit our website.

Our business goal is to help company's monitor business unit and supplier compliance to relevant industry standards and guidelines. Why? Because we wanted to build a comprehensive database of requirements across industries, compliance domains (e.g. security, quality, risk) and supply chain business partners (e.g. foreign manufacturers, shippers, domestic suppliers).

While the forms are not our main business, the difference between our forms and those generated by others is, we believe, that the compliance requirements, regardless of the source, were restated, reworded, and regroup from the perspective of supply chain professionals.

We did not "farm out" the down-and-dirty work required to generate the form sets. We generated the forms; and, in fact, we had to redo more than one form set more than one time as we refined what we needed to do to build an internally consistent, comprehensive, and cross-standard database of requirements.
